The following is a list of costs for common services that Family Acuity offers. Contact us today for a customized Fertility Plan.

Family Acuity begins this process with a free 30 minute personal consultation via telephone. During this consultation you will help Family Acuity determine your specific needs and desires so that we can best determine how to assist you. Family Acuity will consult on the different options available and help assist in the best way to proceed.

A detailed questionnaire will be made available. Your answers to the questionnaire will provide us details on how best to map out a fertility plan specific to your needs. It will ensure that we understand your particular situation and address any questions or concerns you may have. Your Fertility Plan will be personalized based on your desired timeline, financial situation, and specific needs and goals.

The Fertility Plan will then be reviewed with you either in person (when applicable) or via telephone or email.

Consultation: $700

We offer you the option to have Family Acuity review your egg and/or sperm donor selections. We will review medical histories, donation history, and other pertinent information for to 7 potential candidates and provide you with all of the information we receive.

Donor Review for up to 7 Candidates: $500
Additional Reviews: $75 each

If you decide to use a surrogate, there are unique considerations that you may wish to discuss with Family Acuity. A Surrogate Consultation typically lasts between one and two hours, during which we will answer all of your questions and concerns, even ones you didn’t think to ask! The consultation will include a discussion of having a friend or family member as your surrogate, the difference between using a clinic and an independent surrogate, how the proximity of your surrogate may affect the process, state laws that may have an effect on your decision, and other pertinent topics. We will also review up to 3 candidates that you have chosen and provide you with recommendations based on our review.

Surrogate Consultation: $600
Additional Candidate Reviews: $125 each
